Below you will find our downloadable new patient forms, which may be printed and completed at your convenience. To allow adequate time for review by our surgical and anesthesia team, all completed forms must be received at least one week prior to your child’s scheduled surgery appointment. Completed forms may be submitted by email to [email protected] or by text message to (408) 708-7315.

If you experience any difficulty completing or submitting the forms, please contact our office as soon as possible so we may assist you. Timely receipt of all required documentation is essential for surgical clearance and appointment preparation. Surgery appointments will be postponed or canceled if forms are not received with sufficient time for review.
