Below you will find links to complete our digital new patient forms online through Flex Dental, along with downloadable office policy forms that may be printed and completed at your convenience. To allow adequate time for review by our surgical and anesthesia team, all required forms must be completed and submitted at least one week prior to your child’s scheduled surgery appointment. Digital new patient forms should be submitted through Flex Dental. Completed office policy forms may be submitted by email to [email protected] or by text message to (408) 708-7315.

If you experience any difficulty accessing, completing, or submitting the forms, please contact our office as soon as possible so we may assist you. Timely receipt of all required documentation is essential for surgical clearance and appointment preparation. Surgery appointments will be postponed or canceled if forms are not received with sufficient time for review.
